The Extended Health Team provides care for adult patients with complex health needs.

We take a team approach, using the skills of a range of health professionals to help patients learn to better manage long-term (chronic) health concerns.

How We Help

We aim to improve patients’ quality of life and function by helping them better understand their condition and learning skills to manage their health concerns. We do this in collaboration with the patient’s family doctor and other health care services.

Our team provides education, group sessions, one-to-one treatment and occasionally home visits tailored to a patient’s specific needs.

Our group appointments cover topics such as:

  • education on specific conditions such as chronic pain and Irritable Bowel Syndrome
  • exercise and physical activity
  • understanding nutrition and its relationship to pain
  • improving mental health

Extended Health Team staff

The Extended Health Team operates as an extension of family doctors’ offices. It is not a specialty clinic but we do partner closely with specialty services to provide team support.

Our team of health care professionals include:

  • family doctors
  • registered nurses
  • mental health consultants
  • dietitians
  • social workers
  • occupational therapists
  • physical therapists
  • kinesiologists
  • clinical pharmacists
  • health management nurses
  • medical specialists who consult to our team

When required, we partner with Alberta Health Services specialist services in mental health, psychiatry, chronic pain, geriatric care, rheumatology and gastroenterology.

The Extended Health Team is open Mondays to Fridays. This service is available to patients of Calgary Foothills PCN member physicians and admission is by referral.
