Health Management Nurse

Your health team includes a health management nurse, who works in partnership with your family doctor.

A health management nurse can help you:

  • Set achievable health goals and make healthier lifestyle choices
  • Access support for diabetes, high blood pressure, high cholesterol and other conditions
  • Learn weight management strategies, including ways to manage obesity
  • Receive help to quit smoking or increase physical activity

Request an appointment

There are two ways to request an appointment with a health management nurse:

  • Ask your family doctor if a referral is right for you
  • Call the program coordinator at 1-855-79-CFPCN (23726) or 587-774-9736
