Clinical Pharmacist

 

Your health team includes a clinical pharmacist, who works in partnership with your doctor to review your medications.

A clinical pharmacist can help you:

  • Manage medications for conditions including diabetes, chronic pain, high blood pressure and cholesterol, asthma and COPD
  • Create a personal action plan to achieve your health goals
  • Access information and education about natural medicines and supplements

Request an appointment

There are two ways to request an appointment with a clinical pharmacist:

  • Ask your family doctor if a referral is right for you
  • Call the program coordinator at 1-855-79-CFPCN (23726)¬†or 587-774-9736
