Protecting your heart and keeping it pumping properly is an essential part of your overall health.

Heart disease — like heart failure and heart attacks — is a leading cause of hospitalization in Canada and 90% of Canadians have at least one risk factor for it.

We can help you manage and improve your heart health through changes around diet and exercise, quitting smoking, reducing stress as well as understanding your medications.

How we can help

Working collaboratively with your family doctor, members of our Health Team can help you proactively protect your heart and reduce those risk factors.

Health Management Nurse — can help you set achievable health goals, increase your physical activity, improve your nutrition, support your goal to quit smoking or vaping and assist with sleep or stress management. They can also assist with access to supports for high cholesterol and other conditions.

Clinical Pharmacist — manages medications for conditions, including high blood pressure and high cholesterol, and can help you understand both your prescriptions and natural medications or supplements, as well as suggest medications and over the counter products, information and resources to help you quit smoking or vaping.

Registered Dietitian — through group workshops and one-on-one appointments, our registered dietitians offer nutrition advice and information to help support cardiovascular conditions like heart disease.
