Case Collaboratives are groups of health and community professionals who work collectively to support patients and families with complex physical and mental health and social needs. The collaboratives bring individual professionals who are part of a patient’s care network together to share knowledge, resources and options for their patients.
Calgary Foothills PCN doctors can use case collaboratives to support their patients, depending on where they live. There are two case collaboratives in Calgary and two in Cochrane. In both areas, collaboratives can address the needs of older adults as well as children and youth. Find out more in the video below.
All case collaboratives have a variety of social service and care team agencies at the table to support a patient’s needs. This way, the patient and their family do not have to coordinate these services on their own.
