Moving forward together: A new space for primary care
- News
In April, Cochrane residents will have a new, more welcoming space where they can access free health care support.
Calgary Foothills Primary Care Network has a variety of health professionals located in Cochrane, who can assist with primary care needs ranging from helping to create a care plan for a healthier lifestyle, to helping with medication management and supporting mental health.
These health team members will be moving just around the corner from the existing clinic into a space equipped with larger, private consultation rooms and a multipurpose space. The new space is located at #5, 201 Grande Blvd.
“We are excited about this long-term investment in the Cochrane community,” says Jake Jennings, Executive Director for Calgary Foothills PCN, which manages the Cochrane health team.
“This new location allows us to better support our family doctor members in Cochrane with a more functional space where people can access a wide range of personalized services tailored to their health and well-being.”
From mental health counselling to cholesterol management, all Cochrane residents can access health care support through the Cochrane health team – free of charge. Our team works collaboratively with your family doctor to improve your overall health.
The Cochrane health team includes behavioural health consultants, a health management nurse and clinical pharmacist who can help you:
Bookings with the Cochrane health team can be made through a family doctor referral, by calling 1-855-79-CFPCN (23726) or (587) 774-9736 or booking online. Please note, people looking to see a behavioural health consultant need to have a family doctor in Cochrane who is a member of the Calgary Foothills PCN.
In addition to the local health team, people in Cochrane can access a variety of free health care support – such as online and in-person workshops – through Calgary Foothills PCN.
